This week’s Modern Vascular featured case comes from Dr. Albert Chun, MD, Board-Certified Vascular & Interventional Radiologist for Modern Vascular in Fairfax, VA.


42yo female G2P2 with symptomatic uterine fibroids. Chief complaints included menorrhagia, urinary frequency, and constipation. Of note, the patient had prior DVT which resulted in temporary cessation of OCP’s and initiation of anticoagulation. The patient discussed surgical options with her gynecologist but was interested in uterus sparing treatment with minimal/no interruption in anticoagulation. She was referred for outpatient uterine artery embolization. 

Preoperative MR showed enhancing fibroids (dominant fibroid at fundus, Image A). Pelvic angiogram and selective uterine angiogram (Images B, C) showed corresponding hypervascular fibroids . Intraprocedural superior hypogastric nerve block was also performed (Image D). 

At 2 week follow-up, patient reported no issues at access site. At 3 month follow up, patient reported interval decrease in menorrhagia and urinary frequency. Follow up MR pelvis was scheduled for 6 months post procedure.
