72-year-old male, former smoker, with hypertension, diabetes, hyperlipidemia, and a non-healing ulcer of the left hallux (big toe). During consultation, he was concerned about foot pain and fatigue when walking distances. Angiography demonstrated severe focal stenosis in the distal posterior tibial artery, as well as severe stenosis of the dorsalis pedis and deep perforating arteries.
Following antegrade intervention, there is improved patency of the pedal-plantar loop and demonstrably increased perfusion of the first metatarsal arteries.
Patient returned to clinic with near complete healing of his hallux ulcer and improved walking capacity.
